Perioperative complication risk assessment is a critical skill for physician trainees as it shapes whether they recommend surgery and their approach to shared decision-making with patients. This cross-sectional survey compared internal medicine and surgery residents' gestalt assessment of perioperative risk in complex patients to National Surgical Quality Improvement Program estimates. While surgical residents were more confident in their estimates than internists, both overestimated risk for all procedures.
