This study used Bayesian statistical methodology to evaluate the accuracy of medical error detection by standard retrospective case review. Due to suboptimal inter-rater reliability, use of a single case reviewer tended to overestimate the likelihood of errors, but use of multiple reviewers increased accuracy.
