An error reporting system was successfully implemented at 38 clinical sites in 18 countries staffed by the humanitarian organization Médecins Sans Frontières in response to two safety incidents in which patients were harmed. As a result, several system flaws that contributed to medication errors and delayed diagnoses were identified, and additional safety measures and oversight were implemented.
