Error reporting and analysis is a mainstay of patient safety improvement, but voluntary reporting remains challenging. In this study, home-based palliative care physicians submitted prospective error reports and participated in interviews approximately 1 year after an error reporting system was implemented. High levels of cognitive burden decreased the likelihood of reporting. Framing errors as learning opportunities, not punishment, and knowing the reports would improve patient safety increased the likelihood of reporting.
