Patient safety data can support learning health systems and worldwide improvement. This report discusses a set of indicators to measure safety status worldwide with caveats as to weaknesses in the data. Despite the imperfections of the numbers, a distinct dashboard is provided to enable comparison of 38 countries on the defined measures to assist in prioritization and emphasis of efforts that align with a global improvement plan and collectively improve patient outcomes. The United States appeared 33^rd in the ranking.
