Poor communication at transitions of care can lead to preventable adverse events for hospitalized patients. This commentary describes a strategy to standardize resident physicians' handoffs and reduce medical errors. Elements include: illness severity, patient information, action list, situational awareness and contingency plans, and synthesis by receiver.
