Communication errors during handoffs can have negative effects on patient care. This improvement project sought to design an easily accessible standardized communication tool to enhance safety. The authors describe how their intensive care unit developed a daily goals template attached to a widely used door that aided in information sharing at three stages of team care. The intervention reduced handoff-related communication errors by nearly 50%.
