Transitions of care are vulnerable to medication errors. This article describes the 2020 ACCP Transitions of Care Task Force update to a 2012 white paper highlighting process measures of quality clinical pharmacy services during transitions of care. The Task Force recommended process measures to evaluate medication discrepancies and medication therapy problems as well as outcome measures to evaluate health care utilization (e.g., unplanned readmissions) and clinician and patient satisfaction and engagement.
